When I met Carol for her First Plan, her goal was to visit Palm Beach and meet the Home and Away actors.
During plan implementation, I supported her to engage in services that would help her reach her goal, but she was put on a waiting list.
She came in for Support Connection because the providers didn’t get back to her. Her plan was ending in a few months, so I sat with her making calls until we found the right provider who was willing to support her.
I supported Carol when she met the provider and in telling them what she wanted. She signed the Service Agreement within a week.
The following week, Carol was supported and finally able to visit Palm Beach!
